Job summary

Cabarrus Health & Rehabilitation Center in Concord, NC seeks a qualified RN/LPN to join our nursing team. Shifts are 7am–7pm and 7pm–7am, offering daily responsibilities focused on hands-on resident care and collaboration with healthcare professionals.

You will supervise CNAs, administer medications, and manage care plans to help residents regain and maintain independence, delivering compassionate, high-quality care daily.

Qualifications

• Active, unencumbered LPN license in North Carolina.
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.

Responsibilities

• Supervise, coordinate, and direct the work of CNAs.
• Provide individualized nursing care per each resident's care plan.
• Assist in admission, transfer, and discharge of patients.
• Administer prescribed medications and treatments as ordered by physician.
• Explain procedures and treatments to gain patient cooperation.
• Observe vital signs for deviations from normal and track progress.
• Maintain and document residents' nursing observations and actions.
• Record nursing needs on the care plan to ensure continuity of care.
• Monitor emotional stability and prepare patients for discharge or continuing care.
• Perform additional duties to provide exceptional care.

Cabarrus Health & Rehabilitation Center in Concord, NC is seeking a qualified candidate to join our team as a Registered Nurse (RN), Licensed Practical Nurse (LPN).

Day Shift- 7am to 7pm

Night Shift- 7pm to 7am

What You'll Do

• Supervise, coordinate, and direct the work of nursing assistants (CNAs).

• Provide individualized nursing care to residents according to each resident’s plan of care.

• Assist in the admission, transfer, and discharge of patients.

• Administers prescribed medications and treatments as ordered by the physician.

• Explains procedures and treatments ordered to gain patients’ cooperation and allay apprehension.

• Observe vital signs to detect deviations from normal and gage progress of the patient.

• Maintains patients’ medical record on nursing observation and action taken, such as medications and treatments given, reactions, tests, intake, emission of liquids and solids, and vital signs.

• Records nursing needs of patient on care plan to assure continuity of care.

• Observes emotional stability of patients, expresses interest in their progress, and prepares them for continuing care after discharge.

• Perform other duties as required to provide exceptional care to each resident.

What You Need
