Job summary

Ambrosio Guillen Texas State Veterans Home in El Paso, TX is seeking a Charge Nurse to provide direct nursing care to residents and supervise daily nursing activities. You will ensure regulatory compliance, coordinate admissions and discharges, and lead a team of CNAs on each shift.

The role requires an active RN license and at least one year of supervisory experience in healthcare, with strong communication and leadership skills to support veterans and their families.

Qualifications

  • RN degree or approved program graduate with active state RN license.
  • Minimum of 1 year supervisory experience in healthcare.
  • Trained in rehabilitative and restorative nursing; knowledge of current nursing practices.

Responsibilities

  • Direct and supervise CNAs and nursing staff to ensure proper care delivery and staffing.
  • Manage admissions, transfers, and discharges; greet and orient new residents; coordinate transportation.
  • Maintain accurate records including nurses' notes, census reports, care plans, orders, and incident reports.
  • Administer medications per physician orders; verify resident identity and document accurately.
  • Consult with physicians on care plans; participate in rounds and communicate orders clearly.
  • Review and update care plans daily; involve residents in care planning and ensure staff adherence.

What You'll Do

As a Charge Nurse, you will provide direct nursing care to residents and supervise daily nursing activities of staff and CNAs in compliance with all applicable regulations. Responsibilities include:

  • Direct and support CNAs and nursing personnel to ensure compliance with facility policies, proper care delivery, and adequate staffing on each shift.
  • Manage admissions, transfers, and discharges; greet and orient new residents; ensure room readiness; coordinate transportation and notify families as needed.
  • Maintain accurate records including nurses' notes, census reports, care plans, physician orders, medication records, and incident reports.
  • Administer medications and treatments per physician orders; verify resident identity, document accurately, and ensure proper narcotics handling.
  • Consult with physicians on care plans and changes in condition; participate in rounds and ensure timely communication of orders and concerns.
  • Review and update care plans daily; ensure all staff are informed and delivering care as outlined; involve residents in care planning.
  • Provide direct nursing services including catheterization, wound care, feeding tubes, vital signs, rehab, restorative care, and end-of-life care.
  • Maintain clean and safe environments; enforce safety protocols; participate in infection control programs and ensure proper use of PPE.
  • Participate in state/federal inspections; maintain regulatory compliance in documentation, treatment, and resident rights.
  • Meet with residents and families to address concerns; investigate complaints or allegations; maintain detailed records of follow-up actions.
  • Collaborate with dietary, therapy, and other departments to ensure cohesive resident care and adherence to dietary and therapeutic plans.
  • Participate in and lead in-service trainings, orientations, and continuing education on topics such as OSHA, infection control, and safety standards.

Qualifications

  • Education & Licensure: Nursing degree or approved RN program graduate with an active, unencumbered state RN license.
  • Experience: At least one year of supervisory experience in a healthcare setting.
  • Clinical Knowledge: Trained in rehabilitative and restorative nursing; familiar with current nursing practices and procedures.
  • Regulatory Awareness: Knowledgeable of long-term care laws, regulations, and guidelines.
  • Communication Skills: Able to read, write, speak, and understand English effectively.
  • Leadership & Interpersonal Skills: Capable of making independent decisions and leading staff with professionalism and tact.
