Job summary

Focused Post Acute Care Partners FPACP is seeking a Charge Nurse to join our team in delivering high-quality nursing care to residents in a long-term care setting. The Charge Nurse will lead licensed and non-professional staff, coordinate patient care, and ensure safety and compliance with physician orders and nursing standards.

Requirements include a valid LVN or RN license, CPR certification, English proficiency, and the ability to follow instructions and manage assignments efficiently.

Responsibilities

  • Provide nursing care to residents as prescribed by the physician and in accordance with standards of nursing practices

  • Maintain resident safety and plan/organize nursing functions of patients in the unit
  • Document nursing history and assess patient condition; develop care plans
  • Ensure infection-control policies and procedures are followed
  • Communicate with patients and families and collaborate with health-care team
