Mesa Vista Health Center in San Antonio, TX is seeking a Charge Nurse – LVN or RN to lead direct care teams, supervise nurse aides, and ensure safe, compassionate resident care. The ideal candidate is a licensed RN or LVN in good standing with strong clinical assessment, documentation, communication, and emergency response skills.

You will implement care plans, administer medications, transcribe orders, and contribute to shift reports.

• Direct Care Teams: Provide guidance to Nurse Aides to ensure appropriate care for residents.
• Administer Medications: Properly and safely administer resident medications as prescribed.
• Implement Care Plans: Execute resident care plans in compliance with company policies, federal guidelines, and physician orders.
• Perform Treatments: Deliver treatments as required to ensure resident health and comfort.
• Transcribe Orders: Accurately and promptly transcribe physician orders.
• Document Resident Care: Maintain timely and accurate documentation in resident charts and complete required nursing assessments.
• Collaborate During Shift Reports: Participate in shift reports to ensure seamless communication and care continuity.
• Educate Support Staff: Implement nursing policies and procedures, educating staff on the care needs of residents.
• Maintain Safety Standards: Promote a safe working environment by complying with infection control and body mechanic procedures.
• Address Staffing Needs: Respond to staffing shortages by arranging for replacement staff as necessary.
