Creative Solutions In HealthCare seeks a dedicated Charge Nurse - LVN or RN to lead care teams and support resident well-being in our Texas facility. This full-time role requires RN or LVN licensure in good standing, strong clinical and documentation skills, and excellent communication for coordinating shift reports and staff education.

You will oversee care plans, administer meds, perform treatments, transcribe orders, document care, and promote safety with infection control.

Qualifications

• Licensed RN or LVN in good standing.
• Excellent clinical assessment and documentation skills.
• Strong communication and organization of workflows.
• Ability to respond efficiently to emergencies and prioritize tasks.

Responsibilities

• Direct Care Teams: guide Nurse Aides to ensure resident care.
• Administer Medications safely as prescribed.
• Implement Care Plans in compliance with policies and orders.
• Perform Treatments to maintain health and comfort.
• Transcribe Physician Orders accurately and promptly.
• Document Resident Care and complete nursing assessments.
• Collaborate During Shift Reports for continuity of care.

Your Impact as a Charge Nurse - LVN or RN

In This Role, You Will

• Direct Care Teams: Provide guidance to Nurse Aides to ensure appropriate care for residents.
• Administer Medications: Properly and safely administer resident medications as prescribed.
• Implement Care Plans: Execute resident care plans in compliance with company policies, federal guidelines, and physician orders.
• Perform Treatments: Deliver treatments as required to ensure resident health and comfort.
• Transcribe Orders: Accurately and promptly transcribe physician orders.
• Document Resident Care: Maintain timely and accurate documentation in resident charts and complete required nursing assessments.
• Collaborate During Shift Reports: Participate in shift reports to ensure seamless communication and care continuity.
• Educate Support Staff: Implement nursing policies and procedures, educating staff on the care needs of residents.
• Maintain Safety Standards: Promote a safe working environment by complying with infection control and body mechanic procedures.
• Address Staffing Needs: Respond to staffing shortages by arranging for replacement staff as necessary.
