Job summary

Concho Health and Rehabilitation Center is seeking a dedicated Charge Nurse (LVN or RN) to lead direct care for residents, mentor Nurse Aides, and ensure consistent, high‑quality nursing coverage across shifts.

You will administer medications, implement and monitor care plans, transcribe orders, document accurately, and participate in shift reports to maintain seamless continuity of care with physicians and the care team.

Qualifications

• Registered Nurse (RN) or Licensed Vocational Nurse (LVN) in good standing.
• Strong technical, assessment, and documentation skills.
• Excellent communication and the ability to organize workflows.
• Responds efficiently to emergencies with attention to detail.
• Provide direct care guidance and collaborate with the care team.

Responsibilities

• Direct Care Teams: guide Nurse Aides to ensure resident care.
• Administer Medications as prescribed.
• Implement and monitor resident care plans.
• Perform treatments as required for resident health.
• Transcribe physician orders accurately and promptly.
• Document resident care and complete nursing assessments.
• Collaborate during shift reports for continuity of care.
• Educate staff on nursing policies and resident needs.
• Maintain safety by following infection control and body mechanics.
• Address staffing needs by arranging replacements when necessary.

Your Impact as a Charge Nurse - LVN or RN

• Direct Care Teams: Provide guidance to Nurse Aides to ensure appropriate care for residents.
• Administer Medications: Properly and safely administer resident medications as prescribed.
• Implement Care Plans: Execute resident care plans in compliance with company policies, federal guidelines, and physician orders.
• Perform Treatments: Deliver treatments as required to ensure resident health and comfort.
• Transcribe Orders: Accurately and promptly transcribe physician orders.
• Document Resident Care: Maintain timely and accurate documentation in resident charts and complete required nursing assessments.
• Collaborate During Shift Reports: Participate in shift reports to ensure seamless communication and care continuity.
• Educate Support Staff: Implement nursing policies and procedures, educating staff on the care needs of residents.
• Maintain Safety Standards: Promote a safe working environment by complying with infection control and body mechanic procedures.
• Address Staffing Needs: Respond to staffing shortages by arranging for replacement staff as necessary.
