Qualifications

• Current Connecticut LPN or RN license required.
• Prior long-term care experience preferred.
• Strong communication and resident-centered approach required.

Responsibilities

• Plan and manage patient care according to each resident's needs.
• Interview patients and document medical history and physical condition.
• Obtain and record vital signs, including pulse, blood pressure, temperature, and respiration.
• Provide routine nursing care and monitor changes in patient health.
• Review medical charts and communicate updates to the care team.
• Assist physicians and registered nurses with clinical care.
• Communicate clearly with residents and their families.
• Prepare specimens for lab courier pickup.
• Administer medications, injections, and other treatments as ordered.
• Provide wound care and support resident comfort and safety.
• Monitor catheters and other ongoing care needs.
