Job summary

Ohio Living seeks an experienced Clinical Director to oversee residents’ health and wellness across our care team. You will conduct regular visits, assessments, and ongoing communication with care staff to ensure high-quality, compliant patient care.

Qualifications

• Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
• One year experience working as a nurse in a hospital, long term care facility, or other related health care facility.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet preferred.
• Proficiency with electronic medical records (EMR) preferred.
• Must be able to read, write, speak, and understand the English language.

Responsibilities

• Meet with each resident on a regular basis to provide health and wellness checks and answer health-related questions.
• Review all resident information and complete an assessment of the resident prior to move-in, at move in, when changes in the resident’s condition occur, and as required by residence policy and state regulation.
• Review the resident’s medical report prior to move-in to ensure regulatory requirements are fulfilled.
• Order medications and oversee the resident’s medication schedule for residents receiving medication administration services.
• Maintain residents’ wellness files.
• Coordinate resident transfers to and from hospitals or other facilities.
• Assist in coordinating residents’ dietary needs with the Nutrition Coordinator.
• Assess residents and monitor their health.

The Clinical Director oversees and promotes the residents’ health and wellness through regular visits with each resident, resident assessment, and ongoing communication with the resident care team.

Primary Responsibilities

• Meet with each resident on a regular basis to provide health and wellness- checks and answer any health-related questions.
• Review all resident information and complete an assessment of the resident prior to move-in, at move in, when changes in the resident’s condition occur, and as required by residence policy and state regulation.
• Review the resident’s medical report prior to move-in to ensure that the residence can provide for the resident’s individual needs and regulatory requirements have been fulfilled.
• Order medications and oversee the resident’s medication schedule for residents receiving medication administration services.
• Ensure proper documentation for medication assistance/administration
• Develop and maintain an up-to-date individual service plan for each resident.
• Maintain ongoing communication with the resident, resident’s family and physician regarding the resident’s medical needs.
• Coordinate additional health services the resident may need.
• Assist in coordinating resident transfer to and from a hospital or another facility.
• Assist with counseling residents and families.
• Be on call for medical questions and resident emergencies as scheduled.
• Assist in coordinating residents’ dietary needs with the Nutrition Coordinator.
• Review changes in residents’ condition with the Executive Director, and resident’s physician/s
• Assess residents and monitor their health.
• Ensure that all infection control guidelines are met.
• Maintain residents’ wellness files.

Training and Regulatory Compliance

• Facilitate OSHA training and assist employees with OSHA compliance.
• Oversee and assist with training regarding residents’ health and wellness.
• Coordinate state-required training with Executive Director to ensure regulatory compliance.

Communications

• Attend all regular staff meetings and required training sessions.
• Attends various community events.
• Participates in the Manager On Duty program.
• Effectively communicate information pertaining to the residents on a “need to know” basis to other team members according to the residence policies and procedures.
• Notify your supervisor or the Clinical Director if you observe a change in a resident’s condition.
• Accountable for compliance, excellence and delivery of resident care in adherence with organization, local, state and federal regulations.
• Maintains a positive and professional manner toward residents, visitors, families and co-workers.
• Adheres to all policies and procedures of Ohio Living.

Qualifications Education
