As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

The Hospice Chaplain is responsible for providing compassionate spiritual care and support to patients, families, and caregivers in accordance with hospice philosophy. This role delivers spiritual assessments, counseling, and guidance directly or through coordination with community spiritual resources, while collaborating as a member of the interdisciplinary team. The Chaplain contributes to holistic patient care within the scope of practice and in alignment with organizational policies and licensure requirements.
