Job summary

Providence Hospice in Homer Glen, IL, is seeking a part-time Chaplain to provide spiritual, emotional, and religious counseling to patients, families, and staff. You will work 10–19 hours per week across Illinois (Homer Glen area) and Indiana (Lake County) with travel as census grows.

The chaplain will collaborate with a multidisciplinary team to deliver compassionate care, maintain documentation, and support end-of-life needs in line with regulatory standards.

Qualifications

  • Bachelor's or higher degree from an accredited seminary or school of theology (Bachelors of Divinity, Masters of Divinity or equivalent theology degree).

Responsibilities

  • Deliver person-centered spiritual, religious, and emotional care to patients, their family/loved ones and staff.
  • Triages and manages multiple priorities/requests.
  • Provides bereavement services including calls, visits and memorial/celebration of life services.
  • Documents and submit records of pastoral activities per regulations and in patient charts.
  • Must adhere to deadlines for completing documentation.
  • Participates in IDT meetings and collaborates with interdisciplinary staff to promote exceptional care.
  • Able to work evening and weekend hours and travel to patient residences.
  • Report incidents to the administrator and maintain confidentiality of care information.

Hours and Locations

10-19 hours/week, work (1) day in Illinois (Homer Glen and surrounding areas in Will and Cook County) and (1) day in Indiana (Lake County), with possibility of more hours as census grows. Mileage reimbursement $0.76/mile

Providence Hospice, a Providence Life Services community, is seeking a Chaplain, Part time. Our mission is to enhance the lives of those we serve in the name of Jesus Christ. Our Chaplain will carry out the Providence mission and provide spiritual, religious, and personal counseling or guidance to patients, family members, and employees, in accordance with current applicable federal, state, and local standards, guidelines and regulations, and as may be directed by the administrator, to assure that the highest degree of quality patient care can be maintained at all times.

Summary of responsibilities
  • Deliver person-centered spiritual/religious/emotional care to patients, their family/loved ones and staff. This includes but is not limited to spiritual care and support, prayers, end-of-life care, sacramental resources, grief and bereavement support and interdisciplinary care.
  • Triages and manages multiple priorities/requests.
  • Provides bereavement services which can include calls, visits and the attendance of memorial/celebration of life services.
  • Documents and submit records and reports of pastoral activities, per state, federal and Medicare regulations, and prepare necessary information for inclusion in patient’s medical records.
  • Must be able to adhere to mandated deadlines for completing documentation.
  • Aware of and supports the mission of the organization as shown by their actions while on duty.
  • Participates in team (IDT) meetings and collaborates with interdisciplinary staff to promote exceptional care.
  • Able to work evening and weekend hours as needed.
  • Able to travel to where the patients reside within the community.
  • Report all incidents/accidents, complaints, grievances, hazardous conditions, etc., to the administrator immediately.
  • Maintain confidentiality of all pertinent resident’s care information to assure resident rights are protected, including the right of refusal.
Requirements
  • Bachelor’s or higher degree from an accredited seminary or school of theology (Bachelors of Divinity, Masters of Divinity or equivalent theology degree,
  • Minimum of 1 year experience in a health care setting, hospice preferred.
  • Minimum of 5 years’ experience in a pastoral ministry.
  • Familiarity with Microsoft programs and must be comfortable entering information into the medical chart (MAR).
  • Pass a physical, drug screen, 50-pound lift test, fingerprinting for criminal background check and complete a reference check.
  • Valid driver’s license and good driving record.
  • Reliable transportation.
