Job summary

Bristol Hospice Northwest-Crown Point is seeking an experienced Hospice Chaplain for a full-time Day shift in Crown Point, IN 46410. Salary range is $45,000.00 - $58,000.00 per year.

The role focuses on spiritual assessments, developing individualized plans of care, and collaborating closely with the interdisciplinary team. The ideal candidate will have seminary or theology training (BD/M.Div or equivalent preferred) and hospice experience, with willingness to travel between hospice locations as

Qualifications

  • Graduate of an accredited seminary or theology school (BD, M.Div or equivalent preferred).
  • Hospice experience preferred.
  • Willing to travel to hospice locations as assigned.
  • Working knowledge of Microsoft Word, Excel, PowerPoint, and EMR systems.

Responsibilities

  • Assure spiritual assessment of patients and families in the hospice program and ensure timely services.
  • Participate in comprehensive assessments to shape the individualized plan of care.
  • Provide direct spiritual care to patients and families/caregivers.
  • Serve as liaison to community chaplains and spiritual counselors.
  • Document spiritual care services and communicate with the interdisciplinary team.
  • Attend patient care conferences and contribute to care planning.

QualificationsOn an Average Day You Will:
  • Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner
  • Facilitate the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs
  • Provide direct spiritual care to patients and families/caregivers
  • Serve as liaison and support to community chaplains and spiritual counselors
  • Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision
  • Document direct services and ongoing communication with community chaplains and spiritual counselors
  • Attend patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group
  • Provide consultation, education and support to interdisciplinary group on spiritual care
  • Recruit community chaplains, spiritual counselors and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care
  • Provide for funeral or memorial services for patients as requested
  • Plan periodic memorial services to meet the needs of personnel, volunteers and community clergy/spiritual counselors working with the hospice team
  • Assist in supervision of spiritual care volunteers when assigned to patients/families/caregivers
  • Actively participate in quality assessment performance improvement teams and activities
  • Accept and perform other related duties and responsibilities, as required
  • Other duties as assigned
Requirements:
  • Graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree preferred
  • Must have experience working with death and dying, individuals/family/caregiver
  • Hospice experience preferred
  • Working knowledge of Microsoft Word, Excel, PowerPoint, and EMR systems.
  • Must be willing to travel to hospice locations as assigned and be flexible with working hours
