Job summary

View Point Health is a community behavioral health center providing services to individuals with mental illness, substance abuse, and developmental disabilities across Gwinnett, Rockdale, Newton, DeKalb, and Fulton counties. This role focuses on intensive case management and community-based support through ACT teams, with a strong emphasis on recovery and coordinated care.

The position requires a high school diploma or GED with relevant social services experience, plus CPS certification

Qualifications

• High school diploma/GED and three (3) years of job-related experience in a social services-related position; or one (1) year of experience at the lower-level Social Svcs Tech 2 (SST011) or equivalent position.
• Certified Peer Specialist (CPS) Certification or eligible to obtain a CPS certification based on personal lived experience.
• Experience working with severe and persistent mentally ill population, ACT teams, or individuals frequenting ERs, psychiatric hospitals, or crisis units.
• Must be able to lift 20 pounds; valid Georgia driver’s license and motor vehicle record per policy.

Responsibilities

• Provides intensive case management services for consumers with severe and persistent behavioral health disorders.
• Provides services primarily in community settings: home, school and other service settings.
• Conducts face-to-face comprehensive assessments to identify problems and service needs.
• Utilize personal recovery experience to support consumers in meeting daily recovery goals and during crises.
• Function as part of an interdisciplinary team to provide clinical and supportive services.
• Provides and bills only those consumer services authorized in the service plan.
• Arranges appointment schedules to maximize use of work time.
• Monitors consumer progress and compliance with the service plan.
• Writes timely progress notes tied to treatment outcomes.
• Assembles and interprets information to reassess treatment needs.
• Identifies unmet needs and coordinates resources.
