Qualifications

• Associates Degree in Occupational Therapy from an accredited program and licensed in the state of MN.
• One to three years of prior experience in a rehabilitation setting (e.g., acute care, inpatient rehab, outpatient, SNF, long-term care, home health).
• Strong communication skills, attention to detail, and ability to adapt to changing priorities.

Responsibilities

• Assist in Screening & Evaluation: support OT in implementing treatment plans to improve daily living activities.
• Implement Treatment Plans: deliver interventions according to plans and consult with OT as needed.
• Monitor Progress & Document Outcomes: track responses and report to patients, families, staff, and other professionals.
• Support Treatment Planning: help establish goals and resident schedules in collaboration with the care team.
• Educate Patients, Families & Staff: provide education on techniques and adaptive strategies and deliver in-service training.
• Collaborate: coordinate with therapists, nursing, and other professionals for comprehensive rehab programming.
• Maintain Professional Standards: follow documentation, infection control, safety policies, and pursue ongoing education.
