Tender Touch Rehab Services is seeking a Certified Occupational Therapy Assistant - PT to join our skilled nursing facility team. This part-time role offers schedule flexibility and opportunities to contribute to patient-centered rehabilitation in long-term care settings.

As a licensed professional, you will evaluate patients, develop personalized care plans, and collaborate with a multidisciplinary team. We provide ongoing education, growth opportunities, and supportive mentorship to advance

Qualifications

• Must be a licensed Occupational Therapist to apply.
• Degree in Occupational Therapy from an AOTA-accredited program.
• NBCOT certification as an OT, if required by the state.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure all required physician orders are obtained and up to date.

What You'll Do:
