Job summary

Sweetwater Care in Kent, OH is seeking a Certified Occupational Therapy Assistant (COTA) to join our in-house rehab team. You will provide direct patient care in a skilled nursing environment, following plans developed by the Licensed OT, and contribute to improving residents' quality of life. Qualifications

• Active State Licensure as a COTA is required.
• Experience in a skilled nursing setting is preferred.
• Strong communication and teamwork skills.

Responsibilities

• Provide direct therapy under supervision of a Licensed OT.
• Report patient progress and concerns to the OT.
• Deliver therapy based on patient presentation and OT instruction.
• Attend Plan of Care meetings when requested by Rehab Director.
• Record treatment notes and assist with discharge summaries and daily billing.
• Follow state practice acts, code of conduct, and ethics.
• Adhere to company policies and procedures.
• Maintain active COTA licensure.
• Other duties may be assigned as needed.
