Job summary

Personal Touch Home Aides of New York is seeking a Certified Occupational Therapy Assistant (COTA) in Brooklyn, NY. This full-time role requires a valid NY COTA license and offers a dynamic environment with pediatric and adult cases.

The COTA will work closely with the care team to deliver compassionate, high-quality occupational therapy, advancing independence and quality of life for patients across diverse populations.

Qualifications

  • Valid COTA license in NY required.

Responsibilities

  • Assess patient’s functional status and home environment; identify equipment needs.
  • Execute treatment plan and goals; communicate with referring physician and care team.
  • Teach or retrain daily living skills as needed.
  • Fabricate splints and instruct use of adaptive equipment.
  • Train patient in prosthetic/orthotic devices use.
  • Maintain clinical records and submit reports per policy.
  • Coordinate with other personnel for integrated care.
  • Attend in-services and case conferences as required.
  • Identify needs for other home health services and refer appropriately.
  • Instruct family/caregiver on treatment regimens; support discharge planning.

JOB OVERVIEW:

The Certified Occupational Therapy Assistant (COTA) provides compassionate occupational therapy services that help patients improve independence and quality of life while working closely with the care team to deliver personalized, high-quality care.


RESPONSIBILITIES:

1. Assess patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.) assess home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care.


2. Execute treatment program and goals for improved function. Communicates plan of care to referring physician and other Organization professionals.


3. May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury.


4. May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function.


5. May train patient in the use of prosthetic and/or orthotic devices.


6. Maintains appropriate clinical records, clinical notes, summary reports and reports to the physician any changes in the patient’s condition. Submits these records in accordance with policy.


7. Maintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care.


8. Attends and contributes to in-services, case conferences and other meetings as required to ensure coordinated and comprehensive plans of care for the patients of the Organization.


9. Identifies patient and family/caregiver needs for other home health services and refers as appropriate.


10. Instructs patient’s family/caregiver and other health care personnel in patient’s treatment regime when indicated and appropriate.


11. Participates in discharge planning for patient.

QUALIFICATIONS:


  • Licensed/regulated as a Certified Occupational Therapy Assistant (COTA) in the practicing state, if required.


  • Graduate of an ACOTE-accredited Occupational Therapy Assistant program.


  • Strong verbal, written, and organizational skills.


  • Pediatric experience or willingness to work with pediatric patients required.
