At Select Home Health & Hospice, we believe in putting patients first: *“Select the best for your health!”***Job Summary:**Select Home Health is seeking a skilled and compassionate Certified Occupational Therapy Assistant (COTA) to provide in-home therapy services under the supervision of a Registered Occupational Therapist (OT). This role delivers direct patient care based on the established plan of care, monitors patient progress, and supports patients and families in achieving safe independence at home.**Duties/Responsibilities:*** Provide occupational therapy interventions in accordance with the physician-approved plan of care developed and supervised by the Registered OT.* Deliver therapeutic activities and self-care training designed to restore function and improve independence.* Implement orthotic use and adaptive strategies as directed by the OT.* Observe, document, and report patient responses to treatment and any changes in condition to the OT and/or Director of Rehabilitation (DOR) or Director of Nursing (DON).* Educate patients, families, and caregivers on exercise programs, strengthening techniques, transfers, positioning, and activities of daily living.* Assist the OT with ordering, fabricating, or recommending adaptive and assistive equipment.* Participate in client care conferences and interdisciplinary team meetings to ensure coordinated, high-quality care.* Maintain accurate, timely, and compliant documentation in accordance with agency policies and regulatory requirements.* Participate in in-service training and continuing education to remain current with occupational therapy standards and best practices.* Support staff education initiatives as requested.**Physical Requirements*** Ability to sit, stand, bend, lift, and assist patients with mobility and transfers, with or without reasonable accommodation.* Ability to travel to patient homes and drive for extended periods.* Ability to move therapy equipment and supplies as needed.* Regular use of a tablet or electronic device for documentation and communication.**Disclaimer**This description reflects the general nature and level of work for the Certified Occupational Therapy Assistant position.
