Job summary

Granite Peak Home Health is seeking a qualified Occupational Therapy Assistant to join our clinical team. The role requires graduation with OTA certification from an AOTA-accredited program and NBCOT certification, plus current state licensure where applicable.

Two years of experience is preferred. The OTA will work under the Home Health Director and OT team to deliver patient-centered care, develop and update care plans, and participate in patient education and team activities.

Qualifications

• Graduate and meet the requirements for certification as an Occupational Therapy Assistant from an AOTA-accredited program.
• Certification maintained by the National Board for Certification in Occupational Therapy, Inc. (NBCOT).
• Currently certified in the state(s) in which practicing.
• Licensed or regulated as an OTA by the state in which practicing, unless licensure does not apply.
• Two years' experience, preferred.

Responsibilities

• Understands and adheres to agency policies and procedures.
• Improves or minimizes residual physical disabilities of the patient.
• Returns the patient to optimum and productive level within the patient's capabilities.
• Participates with home care team in patient care planning.
• Performs skilled procedures as ordered by physician and per the OT plan of care.
• Consults with OT regarding treatment changes.
• Instructs patients and family in home programs and fine motor exercises.
• Participates in in-service education and presents in-service programs.
• Participates in QAPI activities as assigned.
• Attends patient care conferences as scheduled.
• Prepares medical records and updates care plans for each patient visit as per agency policy.
• Performs services planned, delegated and supervised by the OT.

Principal Responsibilities:

This position reports to the Home Health Director and OT’s

1. Understands and adheres to established Agency policies and procedures.
2. Improves or minimizes residual physical disabilities of the patient.
3. Returns the individual to optimum and productive level within the patient's capabilities.
4. Participates with all other home care personnel in patient care planning.
5. Performs all skilled procedures as ordered by physician and according to the plan of care established by the OT.
6. Consults with OT regarding change in treatment.
7. Instructs patients and family members in home programs and fine motor movement exercises.
8. Participates in in-service education and presents in-service programs as assigned.
9. Participates in QAPI activities as assigned.
10. Attends all patient care conferences as scheduled.
11. Prepares medical records and updates care plans for each patient visit in a timely manner as per Agency policy.
12. Performs services planned, delegated and supervised by the OT.
13. Assists in preparing clinical and progress notes.
14. Participates in educating the patient and family.
15. Performs maintenance therapy services under a maintenance program established by a qualified therapist, when acting within the therapy scope of practice defined by applicable state licensure laws.
