StoneBridge Senior Living is seeking a compassionate Certified Nursing Assistant to support nursing staff in delivering high-quality care to residents. The role emphasizes assisting with daily living activities, mobility, and comfort, while documenting care accurately.

Qualifications

• Certified Nursing Aide in good standing or graduate of state-approved training program and passed competency exam within 120 days of employment.
• High School Graduate/GED preferred.
• Current CPR/BLS certification preferred.
• At least one year of experience as a CNA in long-term care preferred.
• Strong written and verbal communication in an interdisciplinary care setting.

Responsabilités

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists resident with activities of daily living per care plans and policies.
• Assist with lifting, turning, moving, positioning, and transferring residents.
• Coordinate dining room services, meal delivery, feeding assistance, and document intake.
• Deliver nutritional supplements and document intake; report weight changes to nurse.
• Track resident skin condition and report issues to nurse and supervisor.
• Assist with toileting program activities and complete daily flow sheets.
• Notify nurse/supervisor of any factors affecting ability to perform tasks.

HOURS WILL BE 6AM-6PM & WEEKENDS WHEN NEEDED

A Certified Nursing Assistant is responsible for supporting medical staff in administering care to residents with various injuries, illnesses or disabilities. The position requires patience, compassion, and a desire to care for the elderly in a gentle and empathetic manner.

Responsibilities and Duties

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists resident with or performs activities of daily living for resident in accordance with care plans and established policies and procedures.
• Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
• Delivers nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly.
• Assists with weighing residents according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor.
• Assists with tracking the condition of the resident’s skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor.
• Assists nursing staff in carrying out toileting program activities.
• Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor.
• Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e., resident refusal, reassigned to nursing unit).
• Reports changes in a resident’s condition to the resident’s nurse and supervisor.

Requirements

Qualifications and Skills

• Certified Nursing Aide in good standing with the certification board or is a graduate of a state-approved nursing assistant training program and passed a state-approved competency examination (written and skills) within 120 days of employment REQUIRED
• High School Graduate/GED preferred.
• Current CPR/BLS certification preferred.
• At least one year of experience as a certified nursing assistant in long-term care preferred.
• Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary care setting with residents, families, and staff members.
