Job summary

StoneBridge Senior Living in Pocahontas, AR seeks a Certified Nursing Assistant to provide compassionate daily care and support to residents. You will assist with activities of daily living, mobility, meals, and documentation while collaborating with nursing staff to ensure high-quality care.

The role requires CNA certification with preference for 1+ year in long-term care, strong communication, and reliable teamwork within a care-focused environment.

Qualifications

• Must hold Certified Nursing Assistant certification or be a graduate of a state-approved nursing assistant program and pass the competency exam within 120 days of employment.
• High school diploma or GED preferred.
• Current CPR/BLS certification preferred.
• At least one year of experience as a CNA in long-term care preferred.
• Excellent written and verbal communication in an interdisciplinary care setting.

Responsibilities

• Provides supportive services to nursing staff as needed and performs duties as assigned.
• Assists residents with activities of daily living per care plans and policies.
• Aids residents with lifting, transferring, positioning, and mobility devices as needed.
• Coordinates dining room services during meals including setup, delivery, feeding assistance, and intake documentation.
• Delivers nutritional supplements and documents intake.
• Tracks resident weights and reports changes to nurse and supervisor.
• Monitors skin condition and reports concerns to nurse.
• Assists with toileting programs and maintains flow sheets per supervisor instructions.
• Keeps nurse and supervisor informed of factors affecting work performance.

Responsibilities and Duties

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists resident with or performs activities of daily living for resident in accordance with care plans and established policies and procedures.
• Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
• Delivers nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly.
• Assists with weighing residents according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor.
• Assists with tracking the condition of the resident’s skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor.
• Assists nursing staff in carrying out toileting program activities.
• Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor.
• Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e., resident refusal, reassigned to nursing unit).
• Reports changes in a resident’s condition to the resident’s nurse and supervisor.

Qualifications and Skills

• Certified Nursing Aide in good standing with the certification board or is a graduate of a state-approved nursing assistant training program and passed a state-approved competency examination (written and skills) within 120 days of employment REQUIRED
• High School Graduate/GED preferred.
• Current CPR/BLS certification preferred.
• At least one year of experience as a certified nursing assistant in long-term care preferred.
• Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary care setting with residents, families, and staff members.
