Responsibilities
- Receives patient care assignment from the registered nurse (RN) in charge. Initiates delegated tasks based on demonstrated competence in performing nursing activities for the patient age group served.
- Collects and documents data for the admission assessment and reassessment at specified times. Documents and provides data to the RN/LPN for analysis of the patient’s response to care or treatment and for changes in condition.
- Contributes to safety and quality care by relaying information via charting and communicating to team members.
- Informs licensed nurse of changes in patient condition/needs, patient complaints, and patient/significant other educational needs.
- Assists with calls for help, takes vital signs, monitors behavior and physical condition, and assists with baths, ambulation, feeding, skin care, hygiene, and bed-making. Performs dressing changes, enemas, ostomy care, toileting, turning, breathing exercises, catheter care, specimen collection, and intake/output measurement. Uses lifts, tubs, scales and other equipment as needed.
- Prepares patients for discharge and accompanies them upon dismissal as indicated.
- Stocks and maintains supplies and equipment; utilizes time and supplies economically.
- Assists with delegated clerical duties such as answering telephones, relaying messages, transcription of physician orders, and organizing patient records.
- Orients new patients and significant others to the nursing unit.
- Performs mechanical and manual operation of equipment as recommended; recognizes common mechanical problems and initiates corrective action.
- Works in a constant state of alertness and safety.
- Performs other duties as assigned.
Qualifications
- Must hold an active Wisconsin CNA certification
Education
- Completion of an approved nurse aide training program and passing grade on state exam
Experience
Physical Requirements
- Constant standing and walking
- Frequent use of hearing and speech for communication; ability to hear alarms and monitor equipment
- Frequent lifting/carrying and pushing/pulling objects up to 25 lbs (and 25-50 lbs occasionally)
- Frequent reaching, gripping, and keyboard use
- Visual requirements for distances near and far; color recognition
- Occasional bending, stooping, crawling, kneeling, sitting, squatting; repetitive movements
- Occasional driving
