Responsibilities

• Report to RN at shift start and provide handoff at shift end.
• Assist with data collection (vital signs, intake/outtake) and patient observation.
• Assist patients with activities of daily living and mobility as directed by the RN.
• Maintain infection control and monitor for skin integrity and safety.
• Document findings accurately on patient records with signature and role.
• Participate in rounds and coordinate discharge planning with the care team.
