Position Summary:

To provide direct personal care services to clients as indicated in client’s individualized Plan of Care developed by the RN Case Manager or delegate.

Position Responsibilities:
  1. Maintain established Yukon-Kuskokwim Health Corporation and Home Care Department Policies and Procedures.
  2. Perform only services defined in the plan of care and permitted in the CNA scope of services. The Plan of Care should be developed and/or approved by YKHC Home Care RN Case Manager or delegate.
  3. Assist client with basic personal hygiene and grooming, including bathing and dressing and care of the mouth, hair, nail and skin. No nail care should be provided to known diabetics.
  4. Assist client with bladder and bowel routines, which includes helping the recipient to and from the bathroom, assisting the recipient with a bedpan or other toileting procedures; providing general hygiene care of a colostomy, an ileostomy, or an external catheter; giving suppositories that contain no medication; providing digital stimulation; and providing routine care of an incontinent recipient, including use of diapers and protective sheets.
  5. Assist client with self-administered routine oral medication, eye drops, and skin ointments; this assistance may include reminding the recipient and placing a medication within the recipient’s reach.
  6. Assist client with eating, which includes meal preparation essential to meeting the recipient’s health needs.
  7. Assist client with light housekeeping tasks essential to the recipient’s health, including changing the recipient’s bed linens and laundering (only) recipient’s clothing.
  8. Assist client to a medical clinic or physician’s office or on other trips that are reasonably necessary for the purpose of obtaining medical diagnosis or treatment. Prior authorization from RN Case Manger is required.
  9. Assist client with transferring in and out of bed, chair, or wheelchair, and helping the recipient walk with support of a walker, cane, or crutches.
  10. Assist client with simple recommended physical activities and exercises, including active and passive range-of-motion exercises that have been taught to the personal care assistant by a health care professional that is licensed or certified to perform those tasks.
  11. Assist client with positioning or turning of non-ambulatory recipients in a bed or chair.
  12. Assist client with the care of non-sterile dressings and, for uninfected post-operative or chronic conditions, the application of ace bandages and support hose.
  13. Check the client’s temperature (oral, auxiliary, rectal, or ear), pulse (apical and/or radial), blood pressure, and respiration.
  14. If indicated in the plan of care, may assist client with up to two hours a month of assistance with shopping in the vicinity of the recipient’s residence for prescribed drugs, medical supplies, groceries, and other household items required specifically for the health and maintenance of the recipient, including items required by the recipient but also used by other occupants of the recipient’s residence.
  15. Assist client with the use and minor maintenance of respiratory equipment and medically prescribed oxygen.
  16. Assist client with putting on and removing prosthetic devices.
  17. Assist client with the general maintenance of a plaster cast.
  18. Assist client with blood sugar monitor. The CNA may not puncture the skin.
  19. May be responsible, if arranged in advance, for being the emergency back-up worker for a Consumer-Directed client should the primary Consumer-Directed PCA not be available to work or other Home Care Agency-Based provider as directed.
  20. Maintain records for each recipient that the assistant provides personal care services: progress notes, timesheets
  21. Submit progress notes and timesheets per Home Care Policies and Procedures bi-weekly via fax and then mail in originals to the Home Care office.
  22. Perform only services defined in the Plan of Care and permitted in the scope of service.
  23. Telephone RN Supervisor weekly.
  24. Report all incidents and problems to RN Care Manger, Director of Home Care or delegate.
  25. Attend care-planning meetings as directed.
  26. Assist with program projects i.e. outreach activities, health fairs, Senior/Elder community events, and Village events as directed
  27. Assist with emergent department needs as directed.
Position Qualifications:
  • Minimum Education: High School Diploma or equivalent.
  • Minimum Experience: Experience preferred but will train on the job.
  • License, Certification, Registration: Current (and maintain) Certified Nurse’s Assistant License; Current (and maintain) CPR and First Aid
  • Equipment/Tools: Medical equipment as appropriate for position: BP cuff, thermometer, thermometer sheaths, stethoscope, alcohol pads and wipes, sanitizing gel and client equipment on an individualized basis, and personal protective equipment.
  • Specialized Knowledge and Skills: Language Skills: Ability to read, write, and comprehend short correspondence and memos and follow instructions. Ability to write simple correspondence and keep basic records of tasks performed. Ability to effectively present information in one-on-one and small group situations to clients, family and employees of the corporation.
  • Specialized Knowledge and Skills: Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals, Ability compute rate, ratio, and percent and to draw and interpret graphs.
  • Specialized Knowledge and Skills: Reasoning Ability: Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.
