Job summary

Elara Caring is seeking a Certified Nurse Aide to join our home care team in Indiana. You will deliver essential personal care under RN supervision, assist with daily activities, monitor vitals, and help patients maintain independence at home.

Responsibilities include following individualized care plans, documenting changes, and ensuring patient safety. Valid CNA cert and ability to lift up to 50 pounds are required; reliable transportation is needed to travel between patient homes.

Qualifications

• Provide hands-on personal care under the direction of a Registered Nurse.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient's individualized care plan.
• Maintain accurate documentation and protect patient privacy.

Responsibilities

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with daily activities including bathing, dressing, grooming, and toileting.
• Take and record vital signs.
• Help patients move safely and transfer between locations.
• Follow care plans and report changes to the team.
• Maintain a safe, clean home environment.
