Responsibilities

• Provides patients' personal hygiene and assistance with baths, travel to the bathroom and showers.
• Assists with activities of daily living, meals, nourishment, vitals, and intake/output records.
• Ensures patient comfort, transports patients, answers call lights and reports observations.
• Documents actions, maintains work operations and follows policies and procedures.
• Protects patient information and adheres to standards, policies and regulatory requirements.
• Supports SNF community by adhering to standards and continuing education opportunities.
• Maintains knowledge by participating in educational opportunities and hospital initiatives.
