Job summary

Symbii Afton Home Health & Hospice seeks a compassionate Home Health Aide to provide personal care in patients\' homes in Wyoming. You will assist with bathing, dressing, grooming, meals, and mobility, reporting changes to a nurse and following care plans.

Requirements include 18+, CPR certification, a valid driver\'s license, ability to read instructions, and compliance with state/federal laws. This role supports patients safely at home under supervision, with commitment to quality care.

Qualifications

• Meets training requirements per State and Federal laws.
• At least 18 years of age.
• Able to read and follow written instructions and document care.
• Self-directed with minimal supervision.
• Empathy for the needs of ill and frail patients.
• CPR certification is required.
• Licensed driver with a valid, insured vehicle.

Responsibilities

• Provide personal care (baths, back rubs, oral hygiene, shampoos, changing bed linen) as needed.
• Assist patients with dressing, toileting, and hygiene.
• Plan and prepare nutritious meals and assist feeding if necessary.
• Take and record vitals when ordered (temperature, pulse, respiration, blood pressure).
• Assist with ambulation and range-of-motion exercises per plan of care.
• Support self-administration of medications and document accordingly.
• Perform light housekeeping and ensure patient living area is clean and safe.
• Report changes in patient condition to the assigned nurse and follow care procedures.
