The Certified Nursing Assistant provides personal care assistance and home support to maintain a clean, safe environment for the elderly or convalescing patients, enabling them to remain in their own homes. Certified Nursing Assistants work in patients' homes and facilities. They must interact effectively with residents, family members, and staff under all conditions. They communicate regularly with medical staff, nursing personnel, and other units. Certified Nursing Assistants operate under the supervision of a Registered Nurse and the Scheduling Coordinator.

Location: East Falls / Philadelphia PA

Schedule:

• 9 am - 5 pm Sunday through Friday
• 12-6 pm Saturday
• Part-time every other weekend

Primary Job Responsibilities

• Providing frail, ill, or immobile clients with assistance, companionship, and comfort
• Executing the patient care plan and communicating effectively with RN Case Manager
• Monitoring clients' physical, mental, and emotional conditions and reporting changes
• Performing personal hygiene care
• Performing light housekeeping in the patient's living area
• Following nutritional guidelines for patients
• Preparing meals and assisting with eating and drinking
• Assisting with incontinence products or bathroom visits
• Running errands, including shopping for groceries and household items
• Helping clients transfer from beds to chairs or wheelchairs
• With prior approval, accompanying clients on trips such as medical appointments
• Maintaining a clean, safe environment
• Documenting care using the Electronic Visit Verification system
• Communicating schedule needs and changes to Staffing Coordinator
• With proper training, administering medicines and treatments as prescribed

Requirements

• Effective reading, writing, and communication skills
• Understanding written and verbal instructions
• Valid Certified Nursing Assistant License in Pennsylvania
• Passing a job-specific knowledge exam
• Strong organizational skills
• Compassionate attitude towards patient care
• Ability to document actions accurately
• One year of home health experience preferred
• Proof of COVID-19 and Influenza vaccinations or approved exemptions
• Driver's license and auto insurance for driver-designated employees
