Job summary

Optimal Care - Evergreen is seeking a Home Health or Hospice Aide to provide compassionate, hands-on care to patients in their homes across Grand Rapids and surrounding counties. You will help patients maintain dignity and quality of life while supporting individualized care plans in collaboration with nurses and the care team.

Daily responsibilities include assisting with activities of daily living, mobility, meal prep, and comfort measures.

Qualifications

• Must have completed a minimum of 75 hours of classroom and supervised practical training.
• Sixteen hours of classroom must be completed prior to a mandatory 16 hours of practical training.
• Must hold current Nurse Aide certification and be in good standing on the state nurse aide registry.
• Reliable transportation, valid driver's license, and automobile insurance coverage.

Responsibilities

• Provide hands-on home care including bathing, dressing, grooming, toileting, and oral hygiene.
• Assist with meals, mobility, and other daily activities to support independence.
• Collaborate with nurses and interdisciplinary care team to implement care plans.
• Respond to changing patient needs and provide compassionate, respectful care.

As a Home Health or Hospice Aide with Optimal Care, you'll help patients maintain comfort, dignity, and quality of life while supporting their individual needs and goals. Whether assisting patients on their journey toward recovery and greater independence or providing comfort and reassurance during end‑of‑life care, you'll play an essential role in their daily well‑being. Through assistance with bathing, dressing, grooming, toileting, oral hygiene, meal preparation, mobility, and other daily activities, you'll help patients feel safe, supported, and respected.

This role is ideal for a compassionate caregiver who enjoys building meaningful relationships and making a difference through personalized care. You'll work closely with patients, families, nurses, and other members of the interdisciplinary care team to support individualized care plans and respond to each patient’s changing needs. This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours

• Full Time: Monday to Friday, 8 a.m. to 5 p.m., with a weekend on‑call rotation
• Part Time: Three weekdays, 8 a.m. to 5 p.m., with a weekend on‑call rotation
• Per Diem: Flexible weekdays, Monday to Friday, 8 a.m. to 5 p.m., with a weekend on‑call rotation

Required Qualification

• Completed a minimum of 75 hours of classroom and supervised practical training
• Sixteen hours of classroom must be completed prior to a mandatory 16 hours of practical training
• Must hold current certification as a nurse aide and be in good standing on the state nurse aide registry
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Preferred Qualifications

• Experience as a Hospice Aide, Home Health Aide, or Certified Nursing Assistant

Essential Skills and Competencies

• Must be able to read and write in English and follow written and verbal instructions in English effectively
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill in the home or facility
• Is self-directing with the ability to work with little direct supervision
• Secure with issues of death/dying
• Provides a calm manner when in a patient’s home
• Has empathy for the needs of the ill, injured, frail and the impaired

Pay Range

$20 - $24 USD
