Unity Living Center is seeking a Certified Nursing Assistant to join the Long Term Care team in Rochester, NY. You will provide high quality, individualized patient care in collaboration with the nursing staff on days, 7a-3pm, with every other weekend rotation.

Responsibilities include assisting residents with activities of daily living, transport, and documentation, while maintaining a safe, supportive environment. Required NYS CNA program completion and NYS certification; 18+ years old.

Qualifications

• Must complete NYS approved CNA program.
• Must be at least 18 years old.
• Must be registered CNA in NYS.

SUMMARY

As a Certified Nursing Assistant, you will provide high quality, individualized patient care in collaboration with the nursing team. Your ability to take initiative while always assisting and maintaining patient care will be critical in the success of the entire team.

RESPONSIBILITIES

• Patient Care & Service. Provide and/or assist the resident in activities necessary as per individualized care planning; transport to and from activities as needed; provide leisure activities as needed; perform evaluation and reporting; provide resident and family with emotional support
• Safety. Assist in maintaining a clean and safe resident environment
• Communication & Documentation. Support families, direct them to appropriate professional staff; respond to resident requests in a timely manner; receive/give reports to team members/team leaders

REQUIRED QUALIFICATIONS

• Completion of NYS approved certified nursing assistant program
• Minimum age requirement: 18
• Registered Certified Nurse Aide in New York State

PREFERRED QUALIFICATIONS

• Ability to read, write and speak English, ability to follow verbal and written instruction
• Associates degree

Licensed: CNA - Certified Nursing Assistant - NYS Office of Professions

PHYSICAL REQUIREMENTS

Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20–50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask-fit testing, and physical exams are common requirements. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. Any physical requirements reported by a prospective employee and/or employee’s physician or delegate will be considered for accommodations.

PAY RANGE

PAY RANGE: $19.50 - $22.00 CITY: Rochester POSTAL CODE: 14611 The posted pay range is a good faith representation of potential base pay for this position. It may be modified in the future and eligible for additional pay components. Individual annual salaries/hourly rates could vary based on position details and FTE. Pay is determined by considering factors including, but not limited to, experience, relevant qualifications, specialty, internal equity, locations, and contracts.
