Job summary

Collage Home and Community Rehabilitation Partners is seeking experienced CNAs to provide patient-centered in-home care in Austin, TX and surrounding areas. This PRN role focuses on delivering high-quality support in patients' homes and communities, with an emphasis on safety, compassion, and individualized care plans.

Teams tailor treatment plans to each patient’s needs, working in a setting that emphasizes functional living and community integration.

Qualifications

• Experience in healthcare, in-home care, or related settings preferred.
• Ability to lift and move up to 50 pounds.
• Valid driver’s license and reliable transportation.
• Willingness to work different shifts, including nights and weekends.

Responsibilities

• Provide personal hygiene assistance for patients in their homes.
• Assist with medications and ambulation as prescribed.
• Support mobility during home visits and doctor appointments.
• Assist with activities of daily living and basic meals.
• Complete shopping or accompany patients on errands as needed.
• Perform light housekeeping and report any incidents.
• Respond quickly and appropriately in emergencies.
• Support daily living activities and patient comfort.

We are looking for experienced CNA’s who want to provide truly patient-centered care in Austin, TX and surrounding areas - with PRN positions available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.

Areas of responsibility includes but are not limited to:

• Provide personal hygiene assistance for elderly patients or those recovering from surgeries, injuries or accidents by attending to their basic needs, such as showering, bathing, dressing and eating
• Help clients take prescribed medication and assist clients with ambulation
• Help with mobility around the house or outside (doctor’s appointments, walks etc.)
• Assist with personal care and hygiene, plan and prepare meals and help with physical therapy exercises
• Complete client’s shopping or accompany them to successfully do so
• Perform housekeeping duties and report any unusual incidents
• Act quickly and responsibly in cases of emergency
• Offer activities that are essential for daily living by assisting patients with their meals, including serving and other tasks, if necessary

Qualifications

• Enjoy working in a one-on-one setting, eager to improve and continuously learn new skills
• Ability to lift and/or move up to 50 pounds with physical capability to perform job-related duties
• Valid driver’s license and reliable transportation every single day
• Validated ability to act in a compassionate and supportive manner
• Available to work different shifts, including nights and/or weekends
• Prior experience in a healthcare, hospitality, or in-home care environment (minimum of 1 year)
• Willingness to enforce health and safety standards
• Supportive and compassionate
• High School Diploma or equivalent/Experience as a Caregiver, CNA or HHA, or completion of a CNA Program
• Take pride in providing high quality care
• Hoyer Lift Experience (specific to client)
