ATEX Hospice in Corpus Christi, TX is seeking a PRN Hospice Aide to provide personal care to patients in their homes under RN supervision. The position requires empathy, reliability, and availability for approximately 20–25 hours per week.

Responsibilities include direct patient care, safe transfers, documentation of care, infection control, and clear communication with families and staff. A high school diploma and CNA training are preferred; valid driver’s license is required.

Qualifications

  • High School Diploma preferred and nurse aide training/competency evaluated.
  • Current valid driver’s license required.
  • Hospice aide experience or CNA training in a relevant setting is preferred.

Responsibilities

  • Provide direct patient care under the direction of the RN.
  • Assist with safe transfers and ambulation as needed.
  • Document care provided and report changes to patient, family, and supervisor.
  • Maintain infection control principles and a clean, safe home environment.
  • Communicate with patients, family members and staff in a respectful, professional manner.
  • Participate in on-call duties and ensure equipment and supplies are available.

Job Details: Job Location: ATEX Hospice - Corpus Christi - Corpus Christi, TX 78411, Position Type: PRN, Job Shift: Flex, Job Category: Health Care, PRN – Approximately 20–25 hours per week, based on staffing needs.

GENERAL PURPOSE:

Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.

QUALIFICATIONS:
  • High School Diploma Preferred. Has successfully completed one of the following: A)Training program/competency eval as specified in §418.76(b)(c)of the Medicare regulations; B)Competency eval program that meets §418.76( c); C) Nurse aide training/competency eval program approved by the state and listed in good standing on the state nurse aide registry; D) State Licensure program that meets requirements of §418.76(b)©.
  • Must have a current valid driver’s license.
  • At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility or completion of Certified Nursing Assistant or
  • Must be able to read, write, and verbally report clinical information to patients, representatives, and caregivers, as well as to other Agency staff. Demonstrates interest in the welfare of ill and elderly. Successful completion of the Aide Competency Evaluation Skills Checklist that meets§418.76© and written Skills Test.
  • Reliable transportation. Valid and current auto liability insurance
ESSENTIAL JOB FUNCTIONS:
  • Provide direct patient care to patients under the direction of the RN and according to the Aide Plan of Care.
  • Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy.
  • Provide necessary skill to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care.
  • Practice accepted infection control principles. Provide a clean, safe, and comfortable environment.
  • Promote positive, supportive, respectful communication with patient/family and other employees.
  • Provide an environment which promotes respect for patients, privacy and property.
  • Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake.
  • Contribute to the management and efficient operation of the agency and demonstrate effective time management skills.
  • Provide skills necessary to perform treatments and procedures according to agency policy.
  • Promote the agency philosophy and administrative policies to ensure quality of care.
Communication
  • Prepares clinical notes and updates primary physician when necessary and at least every sixty (60) days.
  • Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.
  • Communicates with community health related persons to coordinate the care plan.
Additional Duties
  • Participates in on-call duties as defined by the on-call policy.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Instructs, supervises, and evaluates home health aide care provided every fourteen (14) days.
  • Adhere to all the Company’s policies and procedures.
  • Perform other duties as assigned.
