Qualifications

Responsibilities

• Follows plan of care and attends IDCP meetings as assigned.
• Performs personal care: bathing, nails, grooming, etc., ensuring residents are dressed appropriately.
• Follows infection control procedures and hand hygiene.
• Provides routine skin care and reports skin changes to nurse.
• Maintains resident privacy and HIPAA compliance.
• Assesses comfort levels and documents changes.
• Provides restorative care and routine catheter care.
• Assists with residents with tubes and special equipment.
• Transports residents to activities and services.
• Responds to call bells and keeps water accessible.
• Offers snacks and documents intake.
• Monitors smoking periods as assigned.
• De-escalates residents with verbal strategies and reports to nurse.
• Assists with meal passes and feeding in a timely manner.
• Follows Fall Prevention Protocol.
• Collects lab specimens and communicates changes to nurse.
• Adheres to elopement risk protocols and safety procedures.
• Maintains clean and organized resident rooms.

Job description

QUALIFICATIONS:

• Currently certified in New Jersey as a Certified Nursing Assistant.
• Ability to read, write and comprehend English.
• Long-term care experience preferred.

POSITION SUMMARY:

Under the supervision of a licensed RN or LPN performs nursing services based on the facility's policies and procedures as regulated by the State of NJ, DOH and Federal regulations. Provides assistance to residents with daily routines and other resident activities as needed.

PART I: JOB RESPONSIBILITIES AND STANDARDS

1. Follows the plan of care. Attends IDCP meeting as assigned.

2. Demonstrates proper body mechanics.

3. Provides personal care to residents/guests (bathing, cleaning finger nails, shaving and perineal care, etc.) and assures that resident/guest dresses appropriately.

4. Follows infection control procedures (i.e. hand washing/sorting of linens)

5. Performs routine /special skin care as indicated. Reports skin alterations immediately to nurse.

6. Provides privacy for residents and adheres to HIPAA standards.

7. Inquires about residents/guests comfort/discomfort level on a scale of 0-10.

8. Provides restorative care as indicated.

9. Performs routine catheter care

10. Provides care to residents/guests with tubes and special equipment.

11. Transports resident/guest to and from activities.

12. Answers all call bell and places them in reach of resident/guest.

13. Fills water pitchers with fresh ice water unless contraindicated.

14. Offers snacks to all residents/guests and documents appropriately.

15. Monitors residents during smoking periods when assigned.

16. Demonstrates ability to deescalate resident through verbal strategies, redirections and crisis management and report to nurse immediately.

17. Assist with meal pass and feeding in a timely manner.

18. Follows Fall Prevention Protocol.

19. Collects lab specimens.

20. Reports condition changes/behavioral issues to nurse immediately.

21. Follows elopement risk protocol.

22. Maintains cleanliness and organization of resident/guest rooms including bed making.

23. Accompanies resident/guests during outside medical consultations and out of building activities.

24. Follows safe-handling procedures for personal safety and resident safety at all times.

25. Accurately reflects resident needs on ADL Tracker.

26. Other duties as assigned.

1. Intake and output/hydration.

2. Food intake/calorie count/snack.

3. Weights.

4. Bowel movements.

5. Use of assistive devices.

6. Bath/shower.

9. Observation rounding form (hourly, 30 mins, etc.)

10. Individual statement form post incident

11. Employee Incident/Accident form to Employee Health, Dept/Supervisor by end of shift.
