Job summary

Cantex Continuing Care Network is seeking a Certified Nursing Assistant to provide direct patient care and nursing support under individualized care plans. You will assist patients with routine needs, monitor conditions, and promote safety and independence in collaboration with nursing staff.

The role emphasizes accurate documentation, maintaining a clean and safe care environment, and following procedures for patient transfers and equipment care.

Qualifications

• Current state certification as a Nursing Assistant required.
• Ability to read, write, and speak the English language.
• Ability to lift, move, and transfer patients safely.

Responsibilities

• Provide personal care services to assigned patients in accordance with care plans, safety procedures, and applicable regulations.
• Document patient care activities and observations in accordance with established policies and procedures.
• Promote and support patient independence and quality of life whenever possible.
• Communicate patient status updates and care concerns to the supervising Charge Nurse at the beginning and end of each shift.
• Observe patient conditions, contribute relevant clinical information, and document observations as required.
• Maintain a safe, clean, and hazard-free environment for patients, visitors, and staff.
• Follow established procedures for safe lifting, positioning, transferring, and assisting patients.
• Ensure care-related equipment and mobile devices remain clean, functional, and properly charged, and report malfunctions to the supervisor.
• participate in required in-service education, certification maintenance activities, and Patient Care Management Systems training.
• Perform incidental housekeeping or maintenance tasks necessary to support a clean and safe care environment.
• Serve as a team leader and role model while supporting abuse prevention, patient safety, and quality care initiatives.
• Perform other duties as assigned.

The Certified Nursing Assistant (CNA) works collaboratively with nursing staff to support quality patient care and positive clinical outcomes.
