Job summary

Monarch Healthcare Management is seeking a Certified Nursing Assistant (CNA) to provide high-quality direct care in daily living activities under the supervision of licensed staff. The role emphasizes compassionate care, adherence to the plan of care, and awareness of residents’ mental health and social needs.

The CNA will contribute to an interdisciplinary team, promoting independence and the highest practicable well-being for tenants in Monarch’s Minnesota facilities.

Qualifications

• Current certification and listing on the Minnesota Nursing Assistant Registry.
• Ability to clearly speak and proficiently read and write English.

• Provide direct routine care in activities of daily living under supervision of a licensed nurse or Clinical Manager and in accordance with the plan of care.
• Apply skills in basic nursing, personal care and basic restorative services while supporting cognitive and social needs of tenants.

CNA (Certified Nursing Assistant)

Position Summary:

The CNA is responsible for providing tenants with the highest quality of direct routine care in all activities of daily living, under the supervision of the licensed nurse or AL Clinical Manager and in accordance with the Service Plan, MDH Nursing Assistant Program standards, the Tenant's Bill of Rights and the campus Quality Assurance and Performance Improvement (QAPI) program. According to the Plan of Care for the tenant, CNAs apply skills in basic nursing, personal care, and basic restorative services while understanding the mental health and social service needs of the tenant and being mindful of the needs of cognitively impaired tenants. The CNA is part of the interdisciplinary care team that is dedicated to promoting independence, autonomous decision-making, and facilitating tenants to achieve and maintain the highest practicable physical, mental, and psychosocial well-being.
