Job summary

Elwyn in Rhode Island is seeking a Certified Nursing Assistant to provide essential care in a group living setting. You will support daily activities, monitor health conditions, and help maintain residents' quality of life.

The role requires a CNA license, a high school diploma or GED, and ability to work evenings, weekends, and holidays. Strong communication and teamwork are essential, along with respect for privacy and safety.

Qualifications

• Must have CNA certification and valid license.
• Minimum of 1 year CNA experience preferred.
• Ability to work evenings, weekends, and holidays.
• Strong communication with individuals and families.
• Must have a valid driver's license with at least 2 years driving experience in the U.S.

Responsibilities

• Assist individuals with bathing, dressing, and grooming.
• Monitor medications per care plan and document changes.
• Document assessments and align goals with the Person-Centered Treatment Plan in the EHR.
• Maintain HIPAA confidentiality.
• Assist with transfers and safe movement within the home.
• Prepare meals per dietary requirements and encourage hydration.
• Regularly check vital signs including blood pressure, pulse, and temperature.
• Provide emotional comfort and foster a supportive environment.
• Perform CPR and crisis intervention using agency protocols.
• Maintain current state-mandated training and certifications.
• Housekeeping duties including laundry and cleaning.
• Ensure a clean and safe living space.

POSITION SUMMARY: The Certified Nursing Assistant role focuses on providing essential care and support to individuals within a group living environment. This position will work closely with individuals and their families to ensure their well-being and maintain their quality of life. Responsibilities include assisting with daily activities, monitoring health conditions, and promoting a safe and nurturing environment.

DUTIES AND RESPONSIBILITIES:

• Assist individuals with bathing, dressing, and grooming
• Help maintain personal hygiene and comfort
• Monitor prescribed medications according to the care plan
• Monitor medication schedules and document any changes
• Document assessment and align goals with the Person-Centered Treatment Plan within the EHR (Electronic Health Record) system
• Adhere to HIPAA regulations and maintain confidentiality
• Assist individuals with transferring in and out of bed or chairs
• Ensure safe movement within the home
• Prepare meals based on dietary requirements
• Encourage proper nutrition and hydration
• Regularly check vital signs (such as blood pressure, pulse, and temperature)
• Report any significant changes promptly
• Engage in conversations and provide emotional comfort
• Foster a positive and supportive environment
• Perform cardiopulmonary resuscitation (CPR), crisis intervention and medication administration using agency-trained protocols
• Maintain current state-mandated training and certifications
• Housekeeping duties, including laundry, tidying up, and organizing
• Ensure a clean and safe living space

QUALIFICATIONS, EDUCATION AND EXPERIENCE:

• High school diploma or GED required
• Completion of a certified nursing assistant training program and a valid CNA license certification required
• At least one year of experience as a certified nursing assistant preferred
• Ability to adapt to various home environments
• Effective communication with individuals and their families
• Ability to work evening, weekend and holiday hours according to the team model and an on-call rotation will be required as necessary
• Non-profit human service experience highly preferred
• Demonstrated ability to work effectively as part of a team
• Ability to work in a fast-paced environment and manage competing priorities
• Demonstrated strong attention to detail
• Demonstrated strong time management and organizational skills
• Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
• Must be able to perform CPR and crisis intervention using agency-trained protocols
• Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication
• Basic experience with Microsoft Office applications, including Word and Outlook preferred
• Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record
