Job Summary

As a CNA at Fort Atkinson Health Care Center, you will perform direct patient care, ensuring residents are treated with dignity and respect, supporting their independence, and creating a compassionate environment. You will be the first point of contact for residents and their families, fostering communication and support.

Essential Job Duties & Responsibilities

1. Provide quality nursing care that promotes residents' rights, dignity, and individuality.
2. Support residents' physical, mental, and psychosocial well-being based on individualized care plans.
3. Assist with personal care tasks such as grooming, bathing, feeding, and mobility.
4. Maintain residents' privacy and dignity, interacting warmly and professionally.
5. Understand and uphold residents’ rights, reporting any violations or abuse promptly.
6. Maintain a safe, clean environment, reporting deficiencies as needed.
7. Protect residents’ personal belongings and report missing items.
8. Observe for changes in residents’ condition, reporting findings to licensed staff.
9. Provide skin care to prevent pressure ulcers and skin damage.
10. Respond promptly and courteously to residents’ call bells.
11. Perform tasks such as vital signs, specimen collection, and applying ointments.
12. Assist with resident orientation and transfers.
13. Use proper body mechanics when lifting or moving residents.
14. Communicate effectively with residents, families, and staff.
15. Escort residents to appointments and participate in activities.
16. Use supplies efficiently and follow charge procedures.
17. Document care in CNA records accurately.
18. Follow safety and infection control protocols diligently.
19. Respond to inquiries and requests within policy guidelines.

Other Duties

1. Support administrative tasks and special projects as assigned.
2. Maintain current CNA certification and meet ongoing training requirements.
3. Work rotating shifts, including holidays and weekends, as needed.
4. Practice good personal hygiene and adhere to dress code.
5. Support a safe, respectful, and compliant work environment.
6. Participate in teamwork and communicate tactfully in sensitive situations.
7. Complete in-service training and other duties as assigned.

Qualifications

• High school diploma or equivalent; valid CNA certification in the state.
