This role supports the delivery of safe, effective, and patient-centered care in accordance with the physician’s plan of care, agency policies, and state and federal regulatory requirements. The CNA promotes patient dignity, independence, and comfort while contributing to the overall mission of providing quality care in the home setting.

General Responsibilities

Responsible for carrying out assigned duties that promote patient care, comfort, and safety, while maintaining a clean and supportive environment. Performs all duties in compliance with the agency’s policies, the patient’s individualized plan of care, and all applicable federal and state laws and regulations. Performs other duties as assigned.

Essential Job Responsibilities
Care Delivery
  • Provide or assist with personal care, including bathing, dressing, grooming, oral hygiene, toileting, skin care, and other activities of daily living (ADLs) as directed by the RN Field Case Manager.
  • Assist patients with mobility, transfers, ambulation, and prescribed exercises or positioning.
  • Measure and record vital signs, weight, intake, and output as instructed.
  • Maintain a safe and sanitary environment, changing bed linens and cleaning patient care areas as needed.
  • Observe and report changes in the patient’s condition, behavior, or environment to the supervising nurse promptly.
  • Encourage patient independence and participation in their care plan whenever possible.
  • Provide emotional support and companionship to patients and families.
Documentation/Communication
  • Accurately document care and services provided at the time of the visit in the EMR or designated record system.
  • Report any changes in patient condition, safety concerns, or incidents immediately to the RN Field Case Manager.
  • Communicate professionally with patients, caregivers, and interdisciplinary team members.
  • Maintain confidentiality and adhere to HIPAA and agency standards.
  • Perform other duties as assigned.

Required Experience & Education
  • High school diploma or GED.
  • Current and valid Florida Certified Nursing Assistant certification.
  • Current CPR certification.
  • Valid driver’s license, auto insurance, and reliable transportation.
Florida-Specific Regulatory Requirement

For every Certified Nursing Assistant, the home health agency shall have on file the person’s State of Florida certification. A copy of the screen from the Florida Department of Health website’s Certified Nursing Assistant Information showing the person’s name, address, certificate number, original issue date, expiration date, and status will meet this requirement. (Rule 59A-8.0095(3)(b), F.A.C.)

Compliance, Safety & Patient Rights
  • Adhere to all AHCA Home Health Florida Regulations related to CNA services.
  • Follow agency infection prevention and control policies, including standard and transmission-based precautions.
  • Comply with safety standards, reporting any unsafe conditions or incidents immediately.
  • Respect and protect patient rights in accordance with state and federal regulations.
  • Maintain confidentiality of patient and agency information at all times.
Work Environment Considerations
  • This position operates in patient care environments, which may include private homes, assisted living facilities, or clinical settings. Tasks may require the ability to: Lift up to 50 pounds occasionally and frequently lift or carry up to 25 pounds.
  • Assist with patient transfers and ambulation using proper body mechanics and assistive devices.
  • Tolerate prolonged standing, walking, and physical activity.
  • Respond quickly in emergency situations.
  • Travel throughout the assigned service area daily to visit patients.
OSHA Category 1

– Tasks involve routine exposure to blood, body fluids, communicable diseases or other potentially infectious materials. Must follow universal precautions, infection control procedures and personal protective equipment (PPE) are required.
