Job summary

Ascend Hospice & Palliative Care PA in Montgomery County, PA is hiring a Certified Nursing Assistant (CNA) to provide patient care in a hospice setting. The role involves ADL assistance, mobility support, vital signs, and light housekeeping to maintain patient comfort.

Applicants must hold a State Certified CNA license. Hospice or home health experience is preferred; travel within the area and a valid driver's license are required.

Qualifications

• Must hold a State Certified CNA license.
• Experience in hospice or home health is preferred.
• No experience required but 1 year is preferred.
• Must be willing to travel and have a valid driver's license.

Responsibilities

• Provides or assists in ADL activities and personal care.
• Assists with toileting, transfers, and bed positioning.
• Assists with ROM and prescribed exercises as taught.
• Performs light housekeeping and meal assistance.
• Measures temperature, pulse, and respiration as directed.
• Reports changes to the Hospice Nurse and documents accordingly.
• Assists to remind patient to take medications per regulations.

Now Hiring -Certified Nursing Assistant (CNA) - Montgomery County, PA

Hours: M-F 8:30AM-5PM

ThisCertified Nursing Assistantwill be responsible for, but not limited to:

1. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
2. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
3. Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
4. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
5. Feeds and assists with meals.
6. Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
7. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
8. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Education:

High School Diploma or equivalent

Professional Experience:

• Must have a State Certified CNA license
• Experience in hospice or home health, preferred
• No experience necessary but 1 year, preferred
• Must be willing to travel
• Must have a valid driver's license and reliable transportation
