Job summary

Community Hospice & Palliative Care in Jacksonville, FL is seeking a Certified Nursing Assistant to provide personal care in an adult day center. You will work Monday through Friday under supervision within a interdisciplinary care team.

Responsibilities include activities of daily living support, vital signs, meal service, and documentation in the EMR. A $1000 sign-on bonus is offered and CPR/First Aid or BLS is required.

Qualifications

  • Knowledge of CNA practices and safety protocols.
  • Ability to respond calmly in emergencies.
  • Strong written and verbal communication skills.
  • Maintain confidentiality and HIPAA compliance.
  • Basic computer literacy for EMR documentation.
  • Current CNA certification in state; CPR/First Aid (or BLS) required.

Position Summary

The Certified Nursing Assistant (CNA) provides personal care and support to participants in an adult day center setting, ensuring comfort, hygiene, safety, and active engagement in scheduled activities. This position supports daily operations under the supervision of the Activities Coordinator and functions as part of an interdisciplinary care team. Responsibilities include facilitating activities, delivering direct care in accordance with participant care plans, and maintaining a safe, clean environment.

Location: Ramona Blvd, Jacksonville, FL 32205

Shift: Monday - Friday

$1000 Sign on Bonus

Essential Functions
  • Provide personal care and assistance with activities of daily living to participants.
  • Support and guide participants in organized day center activities.
  • Deliver light housekeeping services and maintain cleanliness of restrooms and shared spaces.
  • Assist with meal and snack service in the day center.
  • Observe and report participant status changes and safety concerns to appropriate staff.
  • Take and record vital signs as needed.
  • Assist with bathing and laundry as requested.
  • Maintain accurate and timely records in the electronic medical record system.
  • Communicate effectively with team members regarding participant care.
  • Participate in staff meetings, required trainings, and quality improvement initiatives.
  • Support and enforce safety protocols, infection control procedures, and HIPAA regulations.
  • Act within the legal and ethical scope of practice for CNAs
  • Perform other duties as assigned.
Qualifications
Knowledge and Critical Skills
  • Knowledge of CNA practices, safety protocols, and use of medical equipment.
  • Ability to respond calmly and effectively in emergency situations.
  • Effective verbal and written communication skills.
  • Strong interpersonal skills to build relationships with participants and staff.
  • Ability to maintain confidentiality and adhere to HIPAA standards.
  • Basic computer literacy.
  • Must possess or obtain current CPR and First Aid certification from a certified provider
Experience
  • Minimum of 1 year of experience working with a frail or elderly population preferred.
  • If less than 1 year of experience, must complete appropriate training from the organization upon hire.
  • Must meet competency standards for the role prior to working independently.
License / Certification
  • Current CNA certification valid in the state of employment.
  • CPR and First Aid or BLS certification required.
  • Valid driver’s license.
  • Level II AHCA background check required.
Education
  • High school diploma or GED equivalent preferred.
Other Requirements
  • Must have medical clearance for communicable diseases and current immunizations before having participant contact.
  • Must maintain a safe work environment and follow all safety, OSHA, and company policies.
  • Participation in emergency preparedness and disaster response as required.
