Responsibilities

• Provide direct patient care, including assistance with activities of daily living (ADLs), such as bathing, dressing, and grooming
• Assist with medication administration, vital sign monitoring, and other medical procedures as needed
• Develop and implement personalized care plans, working closely with our interdisciplinary care team
• Maintain accurate and detailed records of patient care and progress
• Collaborate with our rehabilitation team to support residents' recovery and development of new skills
• Provide emotional support and comfort to residents and their families
• Participate in quality improvement initiatives and contribute to a positive and respectful work environment
