BEACON HOME HEALTH is looking for a Certified Nursing Assistant to join our home health team. The CNA will perform personal care under supervision and report patient observations to the nurse, contributing to quality, continuity and safety of care.

Candidates must complete a state-approved training and competency program, be at least 18, have CPR certification, and have access to transportation for agency-related travel.

Qualifications

• Must have completed a state-approved training and competency evaluation program.
• Must be 18 years or older and able to read and follow written instructions.
• CPR certification must be current and maintained.
• Must have access to transportation for field visits if required.

Responsabilités

• Provide personal care such as baths, oral hygiene, and dressing.
• Assist with activities of daily living and mobility support.
• Observe and report patient condition to the supervising nurse.
• Ensure patient living area is clean and safe, and assist with meals.
• Document care provided and participate in care planning as needed.
• Attend required in-service training and case conferences when scheduled.

JOB SUMMARY

TheCertified Nursing Assistant(CNA)is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The home health aide is responsible for observing patients, reporting these observations and documentingobservations and care performed.The home health aide will be assigned in a manner that promotes quality, continuity and safety of a patient’s care, and only after successful completion of a competency evaluation program.CNA,is also a part of the interdisciplinary team.

DUTIES & RESPONSIBILITIES

Responsibilities of thecertifiednursingassistantinclude, but are not limited to, the following:

Providing personal care including:

• Baths
• Back rubs
• Oral hygiene
• Shampoos
• Changing bed linen
• Assistingpatients with dressing and undressing
• Nail &Skin care to prevent breakdown
• Assistingthe patient with toileting activities
• Keeping patient’s living area clean and orderly, as appropriate
• Planning and preparing nutritious meals.
• Assistingin feeding the patient, if necessary.
• Taking and recording oral,rectaland axillary temperatures, pulse,respirationand blood pressure when ordered (withappropriate completed/demonstratedskillscompetency).
• Assistingin ambulation and exercise according to the plan of care.
• Performing range of motion and other simple procedures as an extensional therapy service as ordered (withappropriate completed/demonstratedskillscompetency).
• Assistingpatientin the self-administration of medication.
• Doingpatient’slaundry, asappropriate.
• Meetingsafetyneeds of patients and using equipment safely and properly (foot stools,
  side rails, etc.).
• Reporting on patient’s condition and significant changes to the assigned nurse.
• Adhering to the Organization’s documentation and care procedures and standards of personal and professional conduct.
• Attend12 clock hours of in-service training annually during a 12-month period.
• As part of the Interdisciplinary Group (IDG), attend scheduled Case Conference meetings to discuss patients in which you areinvolved in.

JOB REQUIREMENTS(Education, Experience, Knowledge, Skills & Abilities)

Must have successfully completed a training and competency evaluation program that is approved by the respective state as meeting all requirements and listed in good standing in the state nurse registry.(Note: Homehealth aide or nurse aide will not be considered completed in a program if there has been a continuous24 monthsor greater period-of-time where the described aide services have not been provided. For lapses of thistime-periodor greater, the individual must complete another training and competency program prior to delivering anyservices)

At least 18 years of age.

Ability to read and follow written instructions and document care given.

Self-directing with the ability to work with little direct supervision.

Empathy for the needs of the ill, injured,frailand the impaired.

Possess andmaintainscurrent CPR certification.

Demonstrates tact,patienceand good personal hygiene.

The ability to drive and/or have access to transportation foragency relatedtravel (e.g.community partners, patients, etc.) is an essential function of the job.Must have andmaintaina valid driver’s license, maintain automobile insurancecoverageand have access to an automobile.

The home health aide will not function in any manner viewed as the practice of nursing according to the State’s Nurse Practice Act.Specifically, the home health aide will not administer medications, takephysician’sordersor perform procedures requiring the training,knowledgeand skill of a nurse, such as sterile techniques.
