Job summary

Responsibilities

• Assist residents with ADLs including bathing, dressing, grooming, toileting, and mobility.
• Observe, document, and report changes in resident condition promptly and accurately.
• Follow individualized care plans and nursing instructions.
• Maintain compliance with all state and federal requirements.
• Adhere to infection control, safety, and HIPAA requirements.
• Support survey readiness through consistent, compliant care delivery.
• Protect resident rights and maintain confidentiality at all times.
• Work collaboratively with skilled nursing staff and interdisciplinary team members.
