Altea Healthcare in DeLand, FL is seeking a Certified Nurse Practitioner (NP) for post-acute care. This full-time role travels between two facilities in the DeLand area, with a swing shift schedule (Tue-Sat or Thu-Mon) and a competitive base salary of $115K-$135K plus uncapped bonuses.

You will diagnose, treat, and manage patients in skilled nursing facilities, collaborate with physicians and nurses, and provide education to patients and families.

Qualifications

• Current NP license or eligibility to obtain.
• All experience levels welcome; training and mentorship provided.
• Interest in geriatrics, internal medicine, or primary care.
• Strong teamwork and communication skills.
• Self-motivated with a patient-first approach.

Responsibilities

• Performing physical exams and reviewing medical histories.
• Ordering and interpreting diagnostic tests (labs, imaging, etc.).
• Diagnosing and managing acute and chronic conditions.
• Prescribing medications and creating treatment plans.
• Collaborating with physicians, nurses, and facility staff.
• Educating patients and families on health conditions and preventive care.
• Documenting patient care accurately and efficiently.

Location: Deland, Florida

Compensation: $115,000 - $135,000 per year + Uncapped Bonus Potential

Job Type: Full-Time

What You’ll Do:
