Job summary

Altea Healthcare in Portland, Oregon is seeking a passionate Certified Nurse Practitioner (PMHNP) to join a rapidly growing organization focused on improving care for patients and providers.

The role involves psychiatric evaluations, follow-ups, urgent consultations, and collaboration with a supervising physician; requires PMHNP certification, state licensure, and DEA/DPS registrations; hybrid schedule 2–3 days on-site.

Qualifications

• Must be PMHNP certified and licensed to practice.
• Experience with adult/geriatric populations.
• Minimum of two years' experience.
• DEA and DPS registration for prescription writing.

Responsibilities

• Provide psychiatric care, including evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs.
• Deliver high-quality, client-centered care.
• Document and navigate EHR independently.
• Maintain strict client confidentiality.
• Create individualized treatment plans after discussion with the mental health care team, reviewing psychosocial data, medical history, and nursing assessment.
• Evaluate patients for higher level of care needs, including hospitalization.
• Educate patients about medications and adherence, and aftercare services.
• Collaborate with supervising physician to improve patient outcomes and care quality.

Job description

We are looking for passionate Certified Nurse Practitioner. The ideal candidate will be motivated by joining a quickly growing organization, and excited to help us improve the care experience for patients and providers. Applicants must be Psychiatric Mental Health Nurse Practitioner certified, licensed to practice, and have experience working with adult/ geriatric populations.

This role will is located in the Portland, OR area**2-3 days per week**.

Responsibilities:

• Provide psychiatric care, including evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs
• Experience in delivering high-quality, client-centered care
• Able to document and navigate EHR independently
• Ability to maintain strict client confidentiality
• Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information
• Evaluate patients for the need for a higher level of care, including hospitalization
• Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare service
• Work closely with Supervising physician to ensure improved patient outcomes and high-quality care

Education and Experience

• Minimum of two (2) years’ experience.
• Master’s degree in Nursing (MSN) from an accredited educational institution is required;
• Psychiatric Mental Health Nursing (PMHNP) certification is required;
• Current state license and board certification
• Current Drug Enforcement Agency (DEA) and Department of Public Safety (DPS) registration for prescription writing
