Job summary

Altea Healthcare is seeking a Certified Nurse Practitioner (NP) to serve as a key clinical provider in a post-acute care and skilled nursing setting. You will diagnose, treat, and guide patients across facilities in the Portland metro area, with no two days the same.

Qualifications

• Certified NP with current licensure or eligibility to obtain licensure.
• Experience in post-acute or skilled nursing care is preferred; training and mentorship available.
• Ability to diagnose, treat, and manage patients in a skilled nursing setting.
• Strong communication and teamwork skills with patients, families, and staff.

Responsibilities

• Perform physical exams and review medical histories.
• Order and interpret diagnostic tests (labs, imaging, etc.).
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and create treatment plans.
• Collaborate with physicians, nurses, and facility staff.
• Educate patients and families on health conditions and preventive care.
• Document patient care accurately and efficiently.

What You’ll Do:

As a Certified NP, you will be a key clinical provider in a skilled nursing facility, diagnosing, treating, and guiding patients to better health. No two days are the same!

Your daily responsibilities include:
