Job summary

RYZE West is seeking compassionate and dependable Certified Nursing Assistants (CNAs) to join our skilled nursing team on the PM shift. CNAs provide direct resident care, assist with daily living activities, and help maintain a safe, comfortable environment for residents.

Responsibilities include assisting with bathing, mobility, vital signs, meals, and documentation while upholding infection prevention and dignity.

Qualifications

• Current, active Illinois CNA certification required.
• Previous CNA experience preferred.
• Skilled nursing or long-term care experience preferred.

Responsibilities

• Assist residents with activities of daily living including bathing, dressing, grooming, toileting and personal hygiene.
• Assist residents with mobility, transfers, positioning, and ambulation.
• Monitor residents' conditions and report changes to the nursing staff promptly.
• Measure and record vital signs as directed.
• Assist with meals, hydration, and feeding as needed.
• Answer call lights promptly and respond to resident needs.
• Maintain accurate documentation of care provided.
• Follow infection prevention procedures and maintain a clean, safe resident environment.
• Provide compassionate care while respecting residents' dignity and privacy.
• Assist with admissions, transfers, and discharges as needed.
• Communicate effectively with nurses and other members of the care team.
• Follow facility policies, procedures, and applicable safety standards.
• Perform other duties as assigned.
