Job summary

Seasons & Crown Hospice, Inc. is seeking a Certified Nursing Assistant (CNA) to provide compassionate, patient-centered care in home or facility settings. You will assist with daily living activities, monitor health changes, and support families at difficult times.

Responsibilities include personal care, vital signs, nutrition assistance, mobility support, and documentation. You’ll work closely with nurses to ensure comfort, safety, and dignity for patients, including pediatric or disabled

Qualifications

• High school diploma or equivalent preferred.
• Completion of an approved CNA program.
• At least 1 year of direct care experience preferred (hospice or home care a plus).
• Ability to lift up to 75–100 pounds and stand or walk for prolonged periods.
• Must pass any required pre-employment screenings and background checks.

Responsibilities

• Assist with bathing, grooming, dressing, toileting, and personal hygiene.
• Change linens, wash laundry, clean living areas, and maintain a safe environment.
• Measure and report vital signs, intake & output, and observe/report health changes.
• Assist with feeding, hydration, ambulation, and patient positioning.
• Administer basic treatments such as enemas, heat/ice packs, and non-sterile dressings.
• Support medication adherence as directed by nurses or physicians.
• Prepare meals in accordance with dietary needs and assist with nourishment between meals.
• Offer companionship, conversation, and emotional support.
• Accompany patients to medical appointments or errands as needed.
• Provide care for pediatric patients or disabled family members when applicable.
• Document care and communicate effectively with the nursing team.
• Assist with post-mortem care as needed in hospice settings.

About the Role:
