Job summary

Lehigh Valley Health Network in the Lehigh Valley area is seeking a Home Health Aide to provide hands-on personal care and activities of daily living as directed by the Registered Nurse or Therapist. You will be part of an interdisciplinary care team delivering compassionate patient-focused services in patients' homes.

Responsibilities include following the plan of care, documenting care provided, and coordinating with clinicians.

Qualifications

• High School Diploma/GED
• 1 year Experience in a healthcare setting as a nursing assistant or nursing aide.
• Builds strong relationships with patients, family members, caregivers, and members of the interdisciplinary care team.
• Knowledge of medical terminology.
• Strong time management skills.
• Ability to drive to patients homes, regardless of weather and traffic conditions, and manage schedule to meet patient needs.
• Ability to work in patient homes.
• Ability to work with and support terminally ill patients and their family members/caregivers.
• American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
• CNA - Certified Nursing Assistant - State of Pennsylvania Upon Hire

• Assumes responsibility for and completes routine and complex assignments for assigned home care/hospice patients within the allotted period of time, including the completion of documentation of patient care provided and communication with other disciplines as appropriate.
• Observes changes in patient functional status and personal care needs; documents observations and reports changes and/or findings to supervising clinician or clinical manager as appropriate.

Home Health/Hospice - must possess and maintain personal car insurance at PA State minimum requirements.

Physical Demands

Lift over 50 lbs. Push/pull up to 140 lbs. Carry 80 lbs. short distances. Frequent standing/walking, squatting, kneeling, crawling, climbing and reaching overhead.
