Job summary

Swan Hospice is seeking a Certified Home Health Aide (CHHA) with hospice experience to join our compassionate care team in New Jersey. You will work with residents in skilled nursing facilities, assisted living facilities, and private homes to provide dignified, patient-centered care.

Ideal candidates have an NJ HHA license, at least one year of hospice experience, and dependable transportation. You will collaborate closely with a multidisciplinary team to uphold our philosophy of kindness,

Qualifications

• Minimum of one year experience working in hospice.
• HHA license in NJ.

Responsibilities

• Assist with prescribed exercises and ROM activities as taught by the Hospice Nurse.
• Provide assistance in toileting; positioning and transfer activities; frequent turning and positioning of a bed bound resident/patient.
• Provide or assist in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Assist or remind the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.
• Assist with light housekeeping of individual's personal unit or room, including changing bed linen and patient laundry.
• Feed and assist with meals.
• Obtain temperature, pulse and respiration as supervised by the Hospice Nurse.
• Report any changes in resident's condition to the Hospice Nurse and document the information with date/time and recipient.

• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information , including the date and time of the report and the person receiving the information.
