Job summary

Swan Hospice is seeking a Certified Home Health Aide (CHHA) with hospice experience to join our team. You will provide compassionate care to residents in facilities and private homes, assisting with ADLs, medication reminders, and routine care under the supervision of our Hospice Nurse.

Ideal candidates have an NJ HHA license, understand hospice philosophy, at least one year of hospice experience, and dependable transportation.

Qualifications

• HHA License in NJ required.
• Hospice experience is required.
• Minimum of one year experience working in hospice.
• Dependable transportation is required.

Responsibilities

• Assists with prescribed exercise and ROM activities as taught by the Hospice Nurse.
• Provides assistance in toileting, positioning and transfer activities, frequent turning and positioning of a bed bound patient.
• Provides or assists in ADL activities: personal hygiene, grooming, mouth, skin care and nails.
• Assists or reminds the patient to take medication under supervision of the Hospice Nurse.
• Assists with light housekeeping of the resident's personal unit or room, including changing bed linen and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respiration as supervised by the Hospice Nurse.
• Reports any changes in resident's condition to the Hospice Nurse and documents information including date/time and recipient.

Job Responsibilities

• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information , including the date and time of the report and the person receiving the information.
